Position Summary |
If you are a self-starter who excels working independently and in a team environment, we have an opportunity for you within our Community Care team in Nelson! Home Health nursing practice supports the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. Working in collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting. This is a relief full time position until September 30, 2021 or return of the incumbent. Shifts for this position are days. Shift times are 08:30 to 16:30. Qualifications |
Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM). Candidates must also hold a valid B.C. driver’s license. Certificates in mental health, palliative care, gerontology care, and wound care would be an asset. Knowledge of adult guardianship, case management and motivational interviewing would be an asset. |
