The patient populations for this position are those with both time limited and continuous home health needs. This position provides both direct client care and care management services.
Your Home Health nursing practice supports the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. Working in collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting.
This position is days and shift times are 08:30 to 16:30, 7 days a week. This does not include STATS.

Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nursing Professionals (BCCNP).
Candidates must also hold a valid B.C. driver’s license.
