F.W. Green Home embodies a resident-centered philosophy designed to support residents’ choices, help people function at their highest level, and provide the best quality of life possible. This facility is a 60 bed facility (53 long term care beds), which is built on three levels, including a ground floor access basement. Two of the beds are Tertiary Geriatric: one is Rehabilitative and the other is Specialized long term care.
In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management.
Days of work for this position are Monday to Friday and shift times are from 08:00 to 12:42, 13:12 to 16:30. This position works at the EKH ED Monday-Friday mornings and at the FWG Home Monday-Thursday afternoons.

Bachelor’s Degree in Social Work from an accredited university.
Two years recent related experience in a healthcare setting working with medically complex clients with
concurrent disorders, or a combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers.
Current valid BC driver’s license.
