Would you like to promote optimal support to clients and help them navigate through the various services available within the community setting? We have the career opportunity for you in our Home Health Nursing team.
As the Home Health Nurse you will support the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. In collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting.
Shifts are 08:00 to 16:00 and this position is rotating days.

Graduation from an approved school of nursing with current practicing registration with the College of Registered Nurses of British Columbia (CRNBC). Candidates must also hold a valid B.C. driver’s license.
