Interior Health is hiring a permanent full time Rural Palliative Social Worker to join our team in the Cariboo. We are seeking an experienced social worker who is comfortable working with the palliative population to join us in this newly created position.
This position is based out of 100 Mile House and provides service to Williams Lake, 100 Mile House, Lillooet, Lytton, Merritt, Barriere and Clearwater.
The Rural Palliative Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.
Shift are Monday to Friday 08:30-16:30 with weekends and statutory holidays off. Travel to remote sites is required and includes flexible extended workdays outside of the regular schedule as well as potential overnights with accommodation provided.
Fleet vehicles may be provided but access to a personal vehicle is required in the event a fleet vehicle is not available.

• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.
**Palliative experience is an asset
**Experience with grief and loss counseling is an asset
