Social Worker | Primary Care

Position Summary

We are hiring a registered Social Worker for the Fruitvale’s Primary Care Network. This position is to provide coverage to September 2027, or return of the incumbent.

Days of work*: Monday to Thursday
Shift times: 08:30 to 16:30

*Hours and days of work and work location are subject to change. This position supports multiple private clinics. Access to a personal vehicle may be required.

About this position:
The Beaver Valley Medical Clinic—Fruitvale’s primary care hub—is part of the Kootenay Boundary Primary Care Network, where a social worker works alongside nurses, occupational therapists, physicians, and allied health professionals such as pharmacists, dietitians, and respiratory therapists to deliver comprehensive, wrap around care. This networked model enables meaningful interdisciplinary collaboration and allows providers to make a tangible impact in a small, tight knit community. Team members enjoy a supportive clinical environment, modern facilities, and the opportunity to build long term relationships with patients while contributing to improved continuity of care across the region.

In this role, you will provide comprehensive assessment, care planning, and treatment coordination for the clients and families you support. You’ll work collaboratively with a multidisciplinary PCN team to deliver direct psychosocial care, counseling, group work, system navigation, and advocacy. You will educate and empower clients, helping them build confidence in their own self management while also connecting them with the specialized and complex care resources they need. As a Social Worker, you play a key role in developing and delivering services that enhance the health, social well being, and overall functioning of the population you serve—making a meaningful and lasting impact in the community.

Qualifications

Bachelor’s Degree in Social Work from an accredited university. Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience. Current full registration with the British Columbia College of Social Workers. Current valid BC driver’s license.
