About the Job
The Licensed Practical Nurse:
• In collaboration with the multidisciplinary team, performs assessments, plans, implements and provides personal and nursing care to clients such as wound care, IV, catheter care and pleurex drains
• Administers oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures
• Operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to organizational policies, procedures, standards and unit specific protocols

TYPICAL DUTIES AND RESPONSIBILITIES:
• Assesses and monitors client/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate, encourages client participation in activities of daily living.
• Establishes, maintains and ends a therapeutic relationship with the client and significant others through the use of interpersonal and interviewing techniques ensuring client choice and autonomy in decision-making and care planning.
• Provide direction to other staff by collegially sharing information, demonstrating and/or providing guidance on clinical or health related tasks, providing incidental or ad hoc input into staff performance, ensuring staff are aware of assignments developed by the Employer, reassigning clinical/health related tasks if necessary.
• Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into interdisciplinary care planning meetings and family conferences. Advocates for the interest of the client and significant others. Collaborates with Community Care Clinician as required.
• Provides maintenance of peripheral intravenous infusions by assessing the intracath insertion site, monitoring flow rate and discontinuing peripheral IV access with an order.
• Performs nursing activities and monitors/evaluates the outcomes. Performs head to toe assessments, airway management, nutrition management, infusion management, catheterizations, applying and changing simple sterile and/or non-sterile dressings, colostomy care, taking/recording vital signs, administering suppositories and enemas. Documents nursing decisions and outcomes and reports problems and/or changes to designated nursing staff. Performs unit specific nursing procedures and interventions as required.
• Diagnoses conditions and communicates client status changes to the designated Community Care Clinician.
• Provides personal care to client including assisting with bathing, dressing, care of skin and hair; changing beds; assisting with toilet needs; and overseeing exercise routines.
• Assists client with meals and maintains intake and output records as required.
• Assists with admissions, discharges, and transfers of clients according to operating policies and procedures.
• Documents and updates all relevant information about care and procedures to achieve client safety and comfort according to unit specific policy and procedure.
• Performs related clerical duties such as answering the telephone, making appointments, ordering supplies and initiating diagnostic requisitions.

This is a Casual opportunity. Casual Employees are not guaranteed hours of work and can range from 0-37.5 hours per week on an as-needed basis.
Shifts of work will be scheduled days, Monday to Friday, rotating from 08:00 to 16:00, 08:30 to 16:30, 08:00 to 18:00 hours.

Qualifications

• Graduation from a recognized program for Practical Nurses and one (1) year of recent, related community, acute care or long-term care experience; or an equivalent combination of education, training and experience
• Current full practicing licensure with the British Columbia College of Nurses and Midwives
• Current valid BC Driver’s License

*Experience in mental health and substance use is in asset*
