This is your opportunity to support vulnerable populations while working on a forward-thinking initiative that’s transforming how home health care is delivered across Interior Health.

About the Role:
As a Home Health Social Worker, you’ll be at the heart of a multidisciplinary care team, working both independently and in collaboration with nurses, rehab professionals, and primary care providers. You’ll support clients by:
• Providing psychosocial care, one-on-one counseling, and group work
• Assisting with system navigation and access to resources
• Advocating for clients’ needs and linking them with complex care supports
• Helping individuals and families build resilience, autonomy, and connection
• Promoting self-management and long-term well-being in the home environment

Whether it’s helping an older adult age in place or supporting someone through a new diagnosis, your work will create a ripple effect of positive change for individuals, families, and communities.

Days of work are rotating 08:00 AM to 4:00 PM. In addition to the scheduled hours, this position includes on-call hours from 04:00 PM to 10:00 PM, in the following rotation:
• Week 1: Monday, Friday, Saturday and Sunday
• Week 2: Thursday
• Week 3: Tuesday and Wednesday

A valid driver’s license is required. You may be required to drive a personal car on mileage and/or be assigned a fleet vehicle.

Qualifications

- Bachelor’s degree in social work from an accredited university.
- Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
- Current full registration with the British Columbia College of Social Workers.
- Current valid driver’s license.
