Position Summary
Interior Health is seeking a dedicated Primary Care Clinical Pharmacist to join the Primary Care Network at East Kootenay Regional Hospital. This position is Relief Full Time (1.00 FTE) working Monday to Friday 08:00 to 16:00. This relief position will provide coverage until the return of the incumbent.
What Will You Work On?
The Primary Care Clinical Pharmacist provides clinical pharmacy services to patients of the primary care network. As a member of an inter-professional team this position works directly with patients and members of the primary care health team to develop patient-specific therapeutic plans based on chart review, patient information and assessment of clinical responses.
Embracing a patient-centered approach, this position provides comprehensive medication management and pharmaceutical care by monitoring the medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions and therapeutic, chemical and laboratory incompatibilities. The Primary Care Clinical Pharmacist identifies and resolves actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team and communicates pertinent findings, recommendations and plans to the patient and appropriate healthcare practitioners.

About This Location:
East Kootenay Regional Hospital (EKRH), located in Cranbrook, is an acute care hospital with a medium size Pharmacy offering inpatient and outpatient services. It operates seven days a week during daytime hours and supports pharmacy services to two satellite hospitals and outpatients in the community.
Cranbrook is situated in the southeastern corner of British Columbia. Cranbrook is the business capital and largest city in the Rocky Mountain Region and is an easy drive to Alberta and the US. Qualifications
• Current registration as a Pharmacist with the College of Pharmacists of British Columbia;
• A minimum 2 years recent, related patient care experience, ideally in a team based primary care setting or equivalent; and
• A valid Class 5 drivers license and access to a personal vehicle for business related purposes.
