Interior Health has an exciting opportunity for a permanent part time (0.99 FTE) Social Worker to join the team at the Cranbrook Urgent Primary Care in Cranbrook, B.C.
Applicable positions may qualify for up to $20,000 as a recruitment incentive.

What Will You Work On?
The successful candidate will be working with a strong, interdisciplinary team including Physicians, Nursing, Physio Therapists, Nurse Practitioners and front desk staff. Further, the candidate will have the opportunity to work with a variety of differing demographics ranging from complex concerns to brief, solution focused support.
Your Role:
The Social Worker provides comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Collaborating with other health care providers and the multidisciplinary team, the Social Worker delivers direct psychosocial care, counseling, group work, system navigation, and advocacy. They facilitate client self-management by linking them to specialized complex care resources and educating them on relevant matters. The Social Worker is dedicated to maintaining or improving the health, social well-being, and functioning of the identified population.
The responsibilities include providing direct social work services and care coordination to individuals, families, and groups. This involves conducting assessments, preparing and implementing care plans, and offering short-term counseling and crisis intervention. The Social Worker also evaluates client responses to interventions, coordinates agency investigations for abuse/neglect, and develops support plans for vulnerable adults. Additionally, they act as a primary case manager, provide education and consultation to other agencies, and engage with resources to support clients. They maintain detailed client records, participate in program development, and contribute to continuous quality improvement initiatives.
Scheduling Information:
Shift types are days and evenings from 08:30 to 20:00.

Qualifications
• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid driver’s license.
