Interior Health has an exciting permanent full-time opportunity for a Bachelor’s Level Social Worker to join their new Primary Care team in Trail, B.C.

This position supports two clinics: Columbia and Riverside. The work location and hour may vary.

What Will You Work On?
In this dynamic role as a Social Worker, you will be at the forefront of delivering comprehensive psychosocial care to individuals, families, and groups. Your expertise will be essential in conducting thorough assessments, creating personalized care plans, and coordinating treatment that addresses the unique needs of each client. Working closely with a multidisciplinary team, you will provide direct counseling, crisis intervention, and facilitate group sessions aimed at empowering clients to manage their own care. Your role will also involve navigating complex systems and advocating for clients to ensure they receive the specialized resources and support they need to thrive.
Beyond direct client care, you will be a vital contributor to the development and enhancement of social work services within the organization. You will bring a social work perspective to team practices, participate in program development, and provide crucial policy and ethical practice recommendations. Your responsibilities will include maintaining detailed client records, evaluating the effectiveness of interventions, and ensuring that the services provided meet the highest standards of care. As a resource to other health professionals and agencies, you will help shape the future of care by sharing your knowledge and advocating for continuous quality improvements that support optimal client outcomes.

Qualifications
• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.
