Position Summary

Interior Health is looking for a Relief Full Time Community Integration Care Coordinator, CH2A. This role is a Team Lead to support the Gateway Crisis Stabilization Unit (5 bed unit), Substance Use Connections Clinical and hospital based MH Crisis Response Team. This role is situated at the Cariboo Memorial Hospital and does not qualify for flexible worksite. The skills ideal for this role is a comprehensive understanding on Crisis Mental Health and Substance Use, Mental Health Act and Mental Health in the Emergency Department. This role is expected to be able demonstrate strong leadership skills.

This is a Relief Full Time position until January 02, 2025 or return of incumbent.

Shift Type & Times: Days Monday to Friday, 07:30 to 15:30

Days Off: Sun, Sat, STAT

What you will do:
The Community Integration Care Coordinator (CICC) practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a client and family centered care model.

The CICC is responsible for the coordination of day to day operations and quality client care within designated clinical areas and supports the seamless movement of clients through the healthcare system and in accessing appropriate resources. The CICC supervises as well as serves as a role model and a resource to staff for clinical decision-making, the clinical assessments of clients and in the counselling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of client care, adequate staffing levels, and a continuous quality improvement environment. Liaises with an interdisciplinary team for the identification, coordination and efficient utilization of resources.

Qualifications

Education, Training and Experience:
Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Advanced preparation in the clinical specialty of the assigned area(s), three years of recent related clinical experience in the assigned practice environment(s) and two years recent supervisory and teaching experience or an equivalent combination of education, training and experience.

Current valid BC Driver’s License.
