Your Role:
This position provides clinical pharmacy services to patients in both acute care and residential care settings. As a member of a multidisciplinary team, you will assess and recommend individualized treatments, act as the primary resource for drug information and education, and provide expert advice to healthcare professionals. You will monitor drug prescribing, distribution, and therapy to ensure the safe, effective, rational, legal, and economic use of drugs. Additionally, you will maintain workload records and statistics, practice patient-centered care by monitoring ongoing drug therapy, and communicate pertinent findings and recommendations to appropriate healthcare practitioners. Your responsibilities will also include interpreting medication orders, entering them into the computer database, dispensing prescriptions, identifying and resolving drug therapy problems, and consulting with physicians on issues such as drug dosage, allergy, incompatibility, interaction, and efficacy.

Qualifications

• Current registration as a pharmacist with the College of Pharmacists of British Columbia.
• Certification of completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR
A combination of at least one year of full time equivalent (FTE) recent relevant clinical pharmacy experience and recognized advanced education courses;
OR
A combination of at least 3 years of full time equivalent (FTE) recent relevant clinical pharmacy experience and an equivalent combination of training, knowledge and experience.
• Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
