What You'll Work On:
In this role, you'll be a key member of our multidisciplinary team, assessing and recommending individualized treatments and serving as the primary resource for drug information and education. You'll monitor drug prescribing, distribution, and therapy to ensure their safe, effective, and rational use. Additionally, you'll practice patient-centered care, meticulously monitoring ongoing drug therapy and communicating vital findings to healthcare practitioners.

Qualifications

• Current registration as a pharmacist with the College of Pharmacists of British Columbia.
• Certification of completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR
A combination of at least one year of full time equivalent (FTE) recent relevant clinical pharmacy experience and recognized advanced education courses;
OR
A combination of at least 3 years of full time equivalent (FTE) recent relevant clinical pharmacy experience and an equivalent combination of training, knowledge and experience.
• Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
