Effective April 1, 2024, the hourly wage for this position will be $32.84 to $41.35.

About the Job:
The Licensed Practical Nurse:
• In accordance with established vision and values of the organization, the Licensed Practical Nurse (LPN) – Transition Liaison practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a patient/client and family centered care model
• Working collegially and collaboratively with multidisciplinary/multi-agency teams, the LPN takes a lead role in planning, implementing, evaluating and coordinating patient/client access and care transition activities between IH facilities and health care services across the continuum of care for all assigned patients/clients with complex care planning and health service needs.
• Acts as a resource to patients/clients/families and various health service teams/professionals in the assessment of patient/client needs and eligibility for appropriate services/resources.
• Operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to organizational policies, procedures, standards and unit specific protocols

This is a PERMANENT FULL-TIME OPPORTUNITY.
Shifts of work will be scheduled days; rotating from 08:00 to 16:00 hours.

Qualifications

• Graduation from a recognized program for Practical Nurses
• One (1) year recent related clinical experience, which includes a combination of Community and Acute Care nursing experience working with the assigned complex patient/client population; or an equivalent combination of education, training and experience
• Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required. Current valid B.C. Drivers License required
