Social Worker (BSW) | Urgent Primary and Community Care Centre (UPCC)

Position Summary

Castlegar B.C. has an exciting opportunity for a relief part time (0.75 FTE) Social Worker to join their team at the Urgent Primary & Community Care Centre. This relief position is until the return of the incumbent.

What Will You Work On?

In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. As the Social Worker you will be responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

This position works a rotating/changing schedule that includes day shifts from 08:00 to 20:00 (including weekends and holidays) for 21 hours per week.

The new urgent and primary care centre team in Castlegar will include general practitioners, registered nurses, social workers, physiotherapists and medical office assistants. The team will provide comprehensive primary care services, including on evenings and weekends. Social Workers at Interior Health advocate for social justice at individual, family, organizational, and societal levels through their professional knowledge, values, ethics and skills.

Qualifications

Education, Training and Experience:

• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.
