The Primary Health Care Nurse provides comprehensive and coordinated nursing care to clients in a primary health care setting through assessment, planning, implementation and evaluation of care delivery. Assists individuals to achieve and maintain optimal health through the provision of information/health promotion, disease prevention and treatment services.
The Primary Health Care Nurse is responsible for working in direct collaboration with an interdisciplinary care team and defined network of Primary Care Physicians to conduct integrated care planning, offer client assessments/clinic visits, and provide ongoing monitoring and self-management support follow-up for clients with multiple chronic conditions, either in an individual or group setting and often directly within the family physician office. The Primary Health Care Nurse works with the interdisciplinary team and other health care providers as required developing effective integrated care planning decision -support tools to assist with overall care coordination.
This is a casual opportunity. Casual means hours are not guaranteed and may vary and shifts may be at short notice. Shift are rotating days with shift times of 08:30 to 16:30.

Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
Advance clinical skills training in the assigned practice area.
One year recent related clinical nursing experience in the assigned practice area, including experience developing and providing formal education to clients, families, the general public, and other health professionals; or an equivalent combination of education, training and experience.
Current valid BC Driver’s License.
