Position Summary

We have a permanent part-time (0.80 FTE) opportunity for a Social Worker to join our Nicola Valley Health Centre / Gillis House in Merritt, B.C.

We are seeking a candidate who is familiar with both long-term care & acute care and applications to Public Guardian and Trust. If you are seeking a unique opportunity in a rural community and willingness to work with a caseload consisting primarily of the frail elderly, we would love to hear from you!

What will you work on?

The Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Shift times are from 08:30 to 16:04, 08:00 to 16:00 hours.

Gillis House and Nicola Valley Hospital, located in Merritt, BC, embodies a client-centered philosophy designed to support clients’ choices, help people function at their highest level, and provide the best quality of life possible. Gillis House is a one storey, stand-alone complex care facility with 74 residential care beds; and Nicola Valley Hospital has 8 beds in Acute Care and 10 beds in Emergency Department.

Qualifications

Education, Training and Experience:

Bachelor’s Degree in Social Work from an accredited university.

Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.

Current full registration with the British Columbia College of Social Workers.

Current valid B.C. driver’s license.
