Position Summary
In accordance with the established vision and values of the organization, the Short Stay Unit (SSU) Nurse Practitioner (NP) cares for patients, usually for less than three months, who require care as follows:
• Convalescent care is provided to clients with defined and stable care needs who require a supervised environment for reactivation or recuperation, usually prior to discharge home, and most commonly following an acute episode of care.
• Hospice care is provided to clients who require support with comfort, dignity and quality of life in the final days or weeks of their lives.
• Respite care provides a client’s main caregiver a period of relief, or to provide a client with a period of supported care to increase their independence.

In this position, the NP performs pertinent physical examinations and clinical evaluations, manages clinical care, performs procedures, orders and reviews diagnostic studies, orders and monitors medications, and participates in short-term and long-term planning of patient care. Joins with members of the inter-professional team in coordinating the transitions in care. A high level of autonomy is expected along with collaboration, consultation and referrals to physicians and other members of the health care team. Requires credentialing and privileging into medical staff of IH NP Regional Department.
As NP you will be most responsible provider (MRP) to the patients admitted. Cottonwoods has 58 bed and Ponderosa has 48 beds. Monday to Friday, 37.5 hours a week. On call expected- group to be determined.
Candidates with 2 years NP clinical experience, SSU, hospitalist or rehab experience are preferred.

TYPICAL DUTIES AND RESPONSIBILITIES:
1. Diagnoses and treats underlying acute and chronic conditions and disorders within the Nurse Practitioner’s scope of practice.
2. Monitors ongoing medical care, orders appropriate screening diagnostic investigations, interprets reports of investigations, and analyzes information to monitor progress and plan treatment.
3. Provides follow-up care; communicates with patients and families about health findings, diagnoses, priorities, outcomes, and prognoses; supports and counsels patients/clients/residents in their responses to diseases, disorders, and conditions.
4. Collaborates and consults with allied healthcare professionals as appropriate to optimize plans for discharge. Participates in family meeting and planning for home.
5. Admits and discharges patients in accordance to organizational policies.
5. Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
7. Participates in research/quality improvement initiatives that contribute to improved patient/client/resident care and advances in nursing, health policy development, and population health.
9. Participates in interdisciplinary staff and nursing education through case presentations; mentoring; role modeling; and facilitating the exchange of knowledge in the classroom, the clinical setting, and the community. Fosters healthcare partnerships.
10. Develops, implements, and evaluates policies and procedures related to nursing, interdisciplinary care, and health system practices.
11. Performs other related duties as required.

Qualifications
Education, Training, and Experience:
• Current registration and exam-qualified as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
• Successful completion of Objective Structured Clinical Examination (OSCE).
• Recent, relevant clinical nursing experience.
