Position Summary
* Note - There is a total of 2.0 FTE available.
In accordance with the established vision and values of the organization, the Rehab Nurse Practitioner (NP) cares for patients in the rehabilitation setting recovering from Stroke, Acute Brain Injuries, Long- stay ICU recoveries, amputations, and other medical conditions. The Rehab NP integrates knowledge, skill and judgment to assess, diagnose, treat and follow up individuals while recovering in rehab.
This role will balance independent practice and collaboration with the Hospitalist, and GP rehab team. There will be a focus on medical management and optimization of acute and chronic illness during the patients rehabilitation stay. The NP will collaborate with allied health to optimize patient care for safe transfer to the community.
Hours are varied from 0700- 1700 Monday to Friday. Shifts are 7.5 hours.
Candidates with 2 years NP clinical experience, hospitalist or rehab experience are preferred.
Privileging and Credentialing required.
TYPICAL DUTIES AND RESPONSIBILITIES:
1. Diagnoses and treats underlying acute and chronic conditions and disorders within the Nurse Practitioner’s scope of practice.
2. Monitors ongoing medical care, orders appropriate screening diagnostic investigations, interprets reports of investigations, and analyzes information to monitor progress and plan treatment.
3. Provides follow-up care; communicates with patients and families about health findings, diagnoses, priorities, outcomes, and prognoses; supports and counsels patients/clients/residents in their responses to diseases, disorders, and conditions.
4. Collaborates and consults with allied healthcare professionals as appropriate to optimize plans for discharge. Participates in family meeting and planning for home.
5. Admits and discharges patients in accordance to organizational policies.
5. Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners and within applicable employer policies and procedures.
7. Participates in research contributing to improved patient/client/resident care and advances in nursing, health policy development, and population health.
9. Participates in interdisciplinary staff and nursing education through case presentations; mentoring; role modeling; and facilitating the exchange of knowledge in the classroom, the clinical setting, and the community. Fosters healthcare partnerships.
10. Develops, implements, and evaluates policies and procedures related to nursing, interdisciplinary care, and health system practices.
11. Performs other related duties as required.

Qualifications
Education, Training, and Experience:
• Current registration and exam-qualified as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
• Successful completion of Objective Structured Clinical Examination (OSCE).
• Recent, relevant clinical nursing experience.
• Experience with care of rehab, older adult population recommended
• ACLS
