Registered Nurse | Heart Function and Cardiovascular Health

Position Summary

We are currently seeking a casual Heart Function and Cardiovascular Health Nurse to join our team at the Kimberley Health Centre.

What will you work on?

Through assessment, planning, implementation and evaluation of quality patient care delivery; The Heart Function and Cardiovascular Health Nurse provides comprehensive and coordinated nursing care to clients as a member of an interdisciplinary health care team, working collaboratively with family practice and/or specialist care physicians, within physician offices, health authority clinics, client’s home and/or other community locations. In individual, group or telehealth settings, the Heart Function and Cardiovascular Health Nurse assists individuals with complex heart failure, cardiovascular and other related chronic conditions to achieve and maintain optimal health through collaborative patient-centered goal setting and action planning, case management, chronic disease related education, health promotion and other clinical care, follow-up monitoring and treatment.

Shifts of work are Monday through Friday, 08:30 to 16:42, and are scheduled on an as needed basis.

• Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
• Successful completion of a recognized Basic Cardiac Course (which includes telemetry). Two years recent clinical experience in acute or chronic coronary/cardiovascular nursing including formal experience in behavioral/lifestyle counseling and identification and management of cardiovascular/chronic disease risk factors and conditions, and one year recent adult learner teaching experience in a formal setting; or an equivalent combination of education training and experience.
• Current valid BC Driver’s License.

Recent ICU experience is an asset
