Under the direction of the Manager, the TL RN’s primary function is to support transitions in care from Royal Inland Hospital to the community. This position provides comprehensive assessment of individuals and is responsible for identifying variances from discharge plans and service needs, as well as resolution of client, caregiver and system issues.
The RN-Transition Liaison acts as a resource to clients/families and various health service teams in the assessment of client needs and eligibility for appropriate resources. This position is responsible for communicating changes in the day to day service needs of the client and liaising with the multidisciplinary team so that changes in capacity and utilization can be anticipated, monitored and alternate strategies developed in order to facilitate the most effective use of health service resources to meet client needs.
This position works at various sites such as Thompson Park Mall, Royal Inland Hospital, Ponderosa Lodge and North Hills Care Centre.
This is a Casual opportunity. Casual means hours are not guaranteed and may vary and shifts may be at short notice. Days of work are rotating, and shift times are 08:00 to 16:00, 08:00 to 16:22, 07:00 to 18:13.

Graduation from an approved School of Nursing plus two (2) years recent related clinical experience which includes a combination of Community and Acute Care nursing experience working with the assigned complex patient/client population; or an equivalent combination of education, training and experience.
Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required. Current valid B.C. Drivers License required.
