Position Summary

We have an exciting opportunity for a Central Intake Home Health Nurse in Penticton B.C at the Penticton Health Centre! This satisfying and complex role focuses on continuous home health services with access coordinator duties and will draw on your LTC-CM nursing experience in the community setting.

The Penticton Health Centre is located across the street from the Penticton Regional Hospital and close to two major shopping centres. In addition to Home Care programs, the Health Centre also houses Public Health Population Health, Mental Health and Addictions services.

The Home Health Nurse practice supports the client’s achievement of safe realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. In collaboration with the client, family and other care providers the Home Health nurse develops, implements and adapts a care plan, delivers direct care, assesses service eligibility and provides care management to help clients manage their own care and navigate through the various services available within the community setting. The Home Health nurse works in consultation with other system partners and bases practice on nursing science and related content from other sciences and humanities.

Days of work are Monday to Friday, 08:00 to 16:00. This is a casual position which means hours of work are not guaranteed. Once hired into a casual role, employees are eligible to apply for internal postings including permanent part-time or full time positions. Education, Training and Experience:
Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
Candidates must also hold a valid B.C. driver’s license.
