Social Worker (BSW) | Urgent and Primary Care Centre (UPCC)

Position Summary

Cranbrook B.C. has an exciting opportunity for a Permanent Part Time Social Worker to join their team at the Urgent and Primary Care Centre. This position works a rotating/changing day and evening schedule that includes shifts from 08:30 to 20:00, 09:30 to 20:00 (including weekends and holidays).

What Will You Work On?

In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management.

As the Social Worker you will be responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

The new urgent and primary care centre team in Cranbrook will include general practitioners, registered nurses, social workers, physiotherapists and medical office assistants. The team will provide comprehensive primary care services, including on evenings and weekends. Qualifications

Education, Training and Experience:

• Bachelor’s Degree in Social Work from an accredited university.
• Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Current valid BC driver’s license.
