Home Health Nurse

Position Summary

If you have great communication skills and the ability to multitask, we have a wonderful career opportunity for you as a Home Health Nurse. This relief part-time position is for time limited and continuous nursing including case management, wound care, parenteral therapy and palliative care and runs to November 7, 2022 or return of the incumbent.

As the Home Health nurse, you will support the client’s achievement of safe, realistic and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. Working in collaboration with the client, family and other care providers you will develop, implement and adapt a care plan, deliver direct care, assess service eligibility and provide care management to help clients manage their own care and navigate through the various services available within the community setting.

Shift times are rotating days and Evenings 08:00 to 16:00, 15:00 to 23:00. There may be opportunities to orientate to other program areas which may include transition liaison, facility access and community clinic. The primary worksite is Thomson Park Mall and our secondary sites are North Hills Care Centre, Royal Inland Hospital and Ponderosa Lodge.

Qualifications

Graduation from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).

Candidates must also hold a valid B.C. driver’s license.
