Interior Health and the Primary Care Network Bridge Care Virtual Clinic has a permanent, full-time opening for an experienced Clinical Dietitian.
Primary Care Networks (PCNs) are community-based networks of health care professionals and clinics who plan and deliver all day-to-day health care needs of a community. Within these networks teams of providers work together closely. In this is way, patients receive better care, by the right person, at the right time, closer to home.
As the Rural Primary Care Network Clinical Dietitian, you will be part of the Bridge Care Virtual Clinic to provide services to attached and unattached patients within the Williams Lake and catchment area. Office space is within in the Cariboo Community Health Centre with opportunity to see clients face-to-face, virtually, and/or occasionally within the community (for example the Alexis Creek health centre). The Bridge Care Virtual Clinic is run/operated by the Central Interior Rural Divisions of Family Practice.
In this position the Clinical Dietitian is responsible for working in collaboration with an inter-professional care team and defined network of Primary Care Physicians, Nurse Practitioners, Nursing and other members of the Allied Health team (i.e. Respiratory Therapist, Physiotherapist, Social Worker, and Occupational Therapist). The goal is to conduct integrated care planning, offer client assessments/clinic visits, and provide ongoing monitoring and self-management support follow-up for clients (i.e. adults, seniors, pediatrics) and/or those with multiple chronic conditions, either in an individual or group setting and often directly within the Family Physician/Nurse Practitioner. You will see clients a couple of times utilizing clinical techniques and knowledge that contribute to providing nutrition care and support to individuals, patients, clients residents and groups.
The Primary Care Network Clinical Dietitian has the support of the Interior Health Clinical Dietitians network across the Cariboo Region. Dietitians come together to support one another and create linkages and connections.
This is a Day shift position on a Monday to Friday schedule 08:00 to 16:00 hours. • Baccalaureate degree in nutrition and dietetics.
• Successful completion of an accredited hospital based dietetic internship program (or equivalent acceptable to the facility).
• Registrant of the College of Dietitians of British Columbia.
• Two year’s of clinical nutrition experience in a health care environment or an equivalent is an asset
• Valid BC Driver’s License.
